The medical record of a patient includes clinical findings, diagnostic test results, pre and postoperative care, and medications to list a few of the healthcare service entries. A medical record is also used for the patient’s future care, drawing on information regarding, for example, past treatments and surgeries, allergies, current medications.

This information can be shared with laboratories, specialists, pharmacies, and other providers to get a complete picture of the patient’s medical history so careful and informed decisions can be made for future care.

Requests for medical records must be made by completing the form below and a proper ID must be shown at the time the records are picked up.

Requests for older records may require additional time and some records may be unavailable.

The Patient Portal is also a 24/7 online tool to access medical records.
